SH-3909 Support avatar height offset
...
Adding a new visual param that allows users to manually adjust an offset for
how far off the ground (+ or -) their avatar's root bone is.
Supports the +-2m range people are used to adjusting in their viewers, but
new implementation should support server-generated appearances.

Converted all gCacheName->getName to getFullName for SLID compatibility
...
Also eliminated notification "ObjectGiveItemUnknownUser" because the
SLURL-based name lookup will always retrieve the user name.
Fixed a bug with ObjectGiveItem where the SLURL would be incorrect
for a group.

DEV-40528 Viewer 2 floods server with AvatarPropertiesRequest messages causing database load. Icon cache was only being used to provide images more quickly, not to suppress network traffic. Made it suppress network traffic, which may result in stale icons occasionally. Converted online/offline tooltip to use LLAvatarTracker. Will port to viewer-2-qa-1. Reviewed with Richard.

Known issues:
* If old Calling Card of person who presents in the friend list is removed - "Do you want to remove ..." dialog is shown, if old Calling Card of non-friend is removed it is removed in the old way without any alerts.
* It is possible to create 2 deep level subfolders under the Calling Cards/Friends/ subfolders. This should be disabled due to Accordion does not support tree hierarchy.
* If friendship is removed when agent is offline, Friend Card is not removed from Inventory.
